Job summary

Optum HouseCalls is recruiting an Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage and other plans in Rhode Island. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Successful candidates will hold current NP or PA credentials with applicable state licensure, be bilingual in Spanish, and possess reliable transportation to perform patient visits in home environments.

Qualifications

• NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• PAs: Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state, with no disciplinary actions.
• Active and unrestricted driver's license with access to reliable transportation.
• Ability to navigate varied home environments and perform assessments.
• If allowed to practice independently, obtain board approval within required timelines.
• New hires must pursue eligibility within 1 month and apply within 3 months.
• Bilingual in Spanish.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history review, vital signs and exams.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Identify urgent situations and intervene appropriately.
• Educate members on diseases, medications and adherence.
• Address social determinants of health and provide referrals as needed.
