Job summary

Optum Home & Community Care, part of the Optum family, is hiring a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare members. The role emphasizes preventive care delivered at patients’ homes, with flexible scheduling and no on-call requirements.

In this field-based position, you will work to close gaps in care, educate patients, and collaborate with primary care providers to improve health outcomes.

Qualifications

• Active and unrestricted licensure in the practicing state with no disciplinary actions.
• NP or PA certification valid for primary care and ability to obtain state licensure by start date.
• Access to reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Review medical history, medications, vitals, and perform appropriate screenings.
• Educate members on disease processes, medications, and care plans; coordinate with PCPs.
• Identify urgent situations and provide referrals as needed.
