Matrix Medical Network is seeking a Nurse Practitioner to perform Comprehensive Health Assessments in patients' homes, reviewing medical history, medication adherence, and risk factors. You will collaborate with physicians, case managers, social workers, and caregivers to close care gaps and improve outcomes.

Responsibilities include conducting geriatric assessments, ordering tests, and providing education and follow-up while traveling to patients in their residences or SNFs.

Formación

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care.
• Current BLS/ACLS/CPR certification.
• Willingness to travel up to 80-90% including overnight travel.
• Valid state driver’s license and automobile insurance coverage.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Coordinate with Primary Care Physician on patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
