Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, capturing medical history, diagnoses, medications, and social factors that impact health. You’ll administer tests, coordinate referrals, and document findings under Matrix guidelines.

The role involves traveling to patients' homes or facilities, with flexible scheduling and a potential sign-on bonus. Active NP license and prescriptive authority are required, plus strong communication skills.

Qualifications

• Master’s degree or commensurate NP experience.
• Current NP licensure in state of practice; prescriptive authority.
• AANP/AACN/ANCC board certification in Adult/Family/GER/AC care specialties.
• Current BLS/ACLS CPR certification.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and care team; follow up with patients.
