Optum Home & Community Care, part of the Optum family, is hiring an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers.

Flexible scheduling is offered with 8am-8pm coverage across a 7-day week; no on-call, no weekends, and no holidays required.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in Louisiana by start date
• Active driver's license and reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments
• Eligibility to practice without Collaborative Agreement where applicable

Responsabilités

• Conduct comprehensive in-home assessments including med history, vitals, and exam
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address gaps in care
• Recognize urgent situations and intervene appropriately
• Educate members on diseases, medications, and adherence
• Address social determinants of health and provide referrals
