Job summary

UnitedHealth Group seeks a HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members. This field-based role focuses on education, gap closure, and collaboration with PCPs to improve outcomes.

Responsibilities include comprehensive exams, documenting diagnoses, and addressing social determinants of health. Requires Louisiana licensure, active ANCC/AANP certification, and reliable transportation for home visits.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in Louisiana.
• Active and unrestricted Driver's license with reliable transportation.
• Ability to navigate varied home environments during assessments.
• If NP independent practice is allowed where you work, obtain licensing board approval within required timelines.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history review, medication reconciliation, vital signs, and physical exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
