Job summary

Optum Home & Community Care is seeking a Family/Adult NP to conduct in-home health assessments in Danville, VA, serving Franklin, Henry, and Pittsylvania Counties. You’ll identify care gaps, educate patients, and collaborate with primary care teams, with 20–40 hours per week and a 8am–8pm schedule across 7 days.

Qualifications

• Active ANCC or AANP certification required.
• Active, unrestricted NP license in the state of practice.
• Active, unrestricted driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform in-home assessments.

Responsibilities

• Conduct in-home health assessments and work within an interdisciplinary care model.
