Job summary

Optum in Danville, VA, part of UnitedHealth Group, seeks a Family/Adult NP to perform in-home preventive health assessments for Medicare Advantage members. You will identify care gaps, educate patients, and coordinate with primary care providers to improve outcomes with flexible scheduling.

This field-based role offers no on-call, weekends, or holidays, and schedules between 20–40 hours per week across 8am–8pm.

Qualifications

• Active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care NP).
• Active, unrestricted NP license in the state of practice (or ability to obtain by start date).
• Active, unrestricted driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform in-home assessments.

Responsibilities

• Conduct in-home health assessments and work within an interdisciplinary care model focused on preventive care and care coordination.
