Job summary

Qualifications

• Active ANCC or AANP certification required.
• Active NP license in the practicing state (or ability to obtain by start date).
• Active, unrestricted driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform in-home assessments.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plans.
• Coordinate with interdisciplinary care teams to improve preventive care outcomes.
• Provide patient education and identify care gaps for tailored care plans.
