Qualifications

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other health plan members.
• Identify and close care gaps; coordinate with primary care providers and interdisciplinary team.
• Educate patients on disease management, medications, and preventive care.
• Address social determinants of health and connect members with resources.
• Work in an independent, field-based role with strong support from the care team.
