Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based role focuses on education, gap closure, and collaboration with PCPs to improve outcomes.

You will conduct comprehensive visits, document findings, and guide care plans. The position offers a 8am-8pm flexible window, no on-call, weekends, or holidays, with a $10,000 sign-on bonus for external candidates.

Qualifications

• Active nursing or PA certification and state licensure required.
• Unrestricted licensure and ability to obtain license in start state.
• Driver's license and reliable transportation for home visits.
• Ability to perform in-home assessments in varied environments.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members.
• Identify and document diagnoses for care management and planning.
• Communicate findings to members' PCPs to close care gaps.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
