Job summary
Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for older adults and at-risk individuals. Providers review medical history, medications, and social factors to close care gaps, with support for licensure, education, and ongoing training.
Responsibilities include conducting adult geriatric assessments, ordering tests, and collaborating with PCPs, case managers, and caregivers.
Qualifications
- NP licensure in state of practice with prescriptive authority
- AANP, AACN, or ANCC board certification in Adult/Adult Gerontology/Family/Acute Care
- Current BLS, ACLS or CPR certification
- 1 year NP experience preferred; new grads encouraged
- Valid state driver’s license
- Automobile insurance coverage for residence state
- Strong computer skills with health/medical record software
- Excellent verbal and written communication skills
Responsibilities
- Conduct adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals, and documentation per Matrix guidelines
- Administer and order point-of-care and lab-based testing as needed
- Collaborate with physicians, case managers, social workers, family members, and caregivers
- Coordinate patient education and follow-up with PCP
- Provide services at the member’s residence (homes or Skilled Nursing Facilities)
