Job summary

Matrix Medical Network seeks NP providers to perform in-home Comprehensive Health Assessments (CHA) for adults and seniors. Visits last up to one hour, reviewing health history, medications, social factors and risks to close care gaps.

Services occur at members' homes or SNFs with collaboration from PCPs. We offer flexible scheduling, competitive pay based on assessments completed, mileage reimbursement and a 401(k).

Qualifications

• Master's Degree or commensurate NP experience and licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care NP specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver's license and automobile insurance coverage.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation as per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and ancillary staff as appropriate.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member's residence (home or Skilled Nursing Facilities).
