Job summary

TriSearch is seeking healthcare professionals to perform wellness visits and in-home assessments for elderly patients. You will conduct comprehensive health assessments, document outcomes, and coordinate with PCPs and care teams. The role offers flexible scheduling and mileage reimbursement, enabling you to balance work with personal commitments.

Qualifications

• Experience performing geriatric/adult health assessments.
• Current RN and NP licensure in the state of practice.
• Board certification in Adult, Family Practice, or Gerontology.
• Must hold CPR certification.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, and treatment planning.
• Collaborate with physicians, case managers, social workers, and other clinical staff.
• Provide documentation per guidelines and maintain PHI compliance.
• Travel to assigned locations to perform home visits and facility visits.
