This field-based position offers flexible scheduling (8am–8pm), with no on-call, weekends, or holidays. Competitive compensation includes bonus options and loan repayment or sign-on incentives.

Qualifications

• NPs must hold active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PAs must hold NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state
• Active and unrestricted driver's license with reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address gaps in care
• Identify urgent situations and intervene appropriately
• Educate members on disease processes, medications, and compliance
• Address social determinants of health and provide referrals as needed
