Job summary

Optum Home & Community Care is expanding its HouseCalls program in Virginia, delivering in-home assessments to Medicare Advantage and other plan members. You will work in an interdisciplinary team, focusing on education, gap closure, and collaboration with primary care providers to improve outcomes.

The role supports flexible scheduling (8am–8pm, 20–40 hours/week), with no on-call, weekends, or holidays. Licensure as NP or PA with relevant certifications is required, plus strong EMR skills and

Qualifications

• Active licensure in the practicing state without disciplinary actions.
• Current ANCC/AANP or NCCPA certification depending on NP/PA track.
• Ability to obtain driver’s license and reliable transportation for home visits.
• Willingness to navigate varied home environments during assessments.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close care gaps and coordinate care.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
