Job summary

Optum Home & Community Care, part of the Optum family, is expanding its HouseCalls program to bring in-home health assessments to Medicare Advantage members. This role focuses on education, gap closure, and collaboration with PCPs in a non-prescribing, field-based setting.

The HouseCalls Advanced Practice Clinician (NP/PA) conducts comprehensive in-home visits, documents diagnoses, and coordinates care while adhering to licensure requirements and delivering high-quality patient experiences.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Ger iatric, or Adult‑Gerontology Primary Care.
• Current NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state without disciplinary actions
• Active and unrestricted driver’s license with access to reliable transportation for home visits

Responsibilities

• conduct comprehensive in-home assessments including medical history, meds reconciliation, vital signs and physical exam
• document diagnoses for care management and treatment planning
• communicate findings to PCPs to address gaps in care
• recognize urgent situations and intervene appropriately
• educate members on disease processes, meds and adherence
• address social determinants of health and provide referrals as needed
