Job summary

Optum Home & Community Care is expanding its HouseCalls program and sending clinicians to patients' homes to perform comprehensive health assessments for Medicare Advantage and other plans. The role emphasizes education, gap closure, and collaboration with primary care providers in an interdisciplinary setting.

As an Advanced Practice Clinician (NP or PA), you will work field-based with no on-call requirements and flexible scheduling between 8am–8pm across a 7-day week, aiming to balance

Qualifications

• 1+ years of clinical experience (family, geriatric, or home health preferred).
• Proficiency with electronic medical records and technology.
• Proven communication skills with geriatric or Medicare populations.
• Fluent in any languages besides English.

Responsibilities

• Conduct in-home health assessments, including medical history, med reconciliation, vital signs, and exams.
• Identify and document diagnoses for care management and planning.
• Communicate findings to PCPs to close care gaps.
• Recognize urgent situations and intervene as needed.
• Educate members on diseases, meds, and adherence; address social determinants and provide referrals.
