Job summary

UnitedHealth Group's Optum HouseCalls team seeks an Advanced Practice Clinician (NP/PA) to perform comprehensive in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geraitric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state.
• Driver's license with reliable transportation for home visits.

Responsibilities

• Conduct in-home assessments and medical history review.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close care gaps.
