Job summary

UnitedHealth Group's Optum HouseCalls is seeking an Advanced Practice Clinician (NP or PA) to perform in-home Medicare Advantage assessments. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve patient outcomes.

Flexible scheduling (20–40 hours/week, CA/WA 24–40), no on-call, no weekends/holidays. Competitive salary with bonus, and travel to patient homes.

Qualifications

• Active certification in NP if applying as NP (ANCC/AANP Family/Adult/Geriatric Primary Care).
• PA candidates require NCCPA certification and state licensure.
• Active and unrestricted license in practicing state.

Responsibilities

• Conduct in-home assessments including history, meds, vitals and exam.
• Identify diagnoses and plan care with PCPs for gap closure.
• Document findings and coordinate referrals; address urgent situations.
