Job summary

UnitedHealth Group, through Optum Home & Community Care, seeks an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes preventive care, education, and collaboration with primary care providers to improve health outcomes.

You will conduct comprehensive assessments, document care plans, and address social determinants of health.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current, unrestricted state licensure to practice
• Active and unrestricted driver’s license and access to reliable transportation for home visits

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members
• Document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address gaps in care
• Identify urgent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
