Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage members. This is a part-time 24-hour-per-week role with a flexible schedule (8am–8pm, 7 days a week), no on-call, no weekends, and no holidays.

The position offers a $10,000 sign-on bonus for external candidates, with compensation in a range that reflects both base pay and bonus.

Qualifications

• Active ANCC or AANP Family/Adult/Geriatric/Adult-Gerontology Primary Care certification.
• Active, unrestricted Texas license by start date.
• Active and unrestricted driver’s license with access to reliable transportation for home visits.
• Ability to navigate varied home environments during assessments.

Responsibilities

• Conduct in-home assessments, including vitals and med history.
• Document diagnoses for care management and planning.
• Communicate findings to patients' PCPs to address care gaps.
• Recognize urgent situations and intervene appropriately.
• Educate patients on diseases, medications and adherence.
• Address social determinants of health and provide referrals as needed.
