Job summary

OptumCare is seeking an Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage members. This field-based role focuses on education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible scheduling within 8am–8pm, no on-call, no weekends, and no holidays. Sign-on bonus or student loan repayment options are available.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• Current NCCPA certification and state licensure for PAs.
• Active, unrestricted state licensure in the practicing state without disciplinary actions.
• Active and unrestricted driver's license with reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments seated/standing/kneeling.
• NPs/PAs must obtain/maintain eligibility as required by state law within set timelines.

Responsibilities

• Conduct comprehensive in-home assessments including history, meds reconciliation, vitals and exam.
• Perform evidence-based screenings and point-of-care testing as appropriate.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to the member's PCP to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
