Job summary

Flournoy Health Systems is seeking a Licensed Practical Nurse (LPN) to provide field-based nursing services in Dougherty County, GA. The LPN will perform medications and treatments per Plan of Care and support clients in their homes.

The role requires travel within the assigned territory, collaboration with RNs and caregivers, and accurate clinical documentation to ensure quality, compliant home health services for medically fragile individuals.

Qualifications

• Current, active LPN license to practice in Georgia.
• Current Basic Life Support (BLS) certification.
• Experience providing care to medically fragile individuals.
• Working knowledge of home and community-based service programs.

Responsibilities

• Provide nursing care, medications, and treatments per Plan of Care and within LPN scope.
• Monitor client health, vital signs, and changes; report findings to RN.
• Perform skilled nursing interventions (medication management, wound care, catheter care, respiratory treatments) as ordered.
• Educate clients and caregivers on medications, treatments, safety, and disease management.
• Complete timely documentation of services, client status, interventions, and changes.
• Coordinate Plan of Care reviews and services with RN and interdisciplinary team.
