Job summary

Flournoy Health Systems is seeking a Licensed Practical Nurse (LPN) to provide field-based nursing services for clients in a home-care setting in Georgia. The LPN will perform supervisory visits, monitor client conditions, and support the RN and care team across the assigned territory.

The role emphasizes direct patient care, education for clients and caregivers, and accurate clinical documentation in line with care plans and regulatory requirements. Travel to client homes is required.

Qualifications

• Current LPN license in Georgia.
• BLS certification required.
• Experience caring for medically fragile individuals.
• Knowledge of home and community-based service programs is a plus.
• Willingness to travel within the assigned territory.

Responsibilities

• Provide nursing care, medications, and treatments per Plan of Care.
• Monitor client health status and report changes to RN/care team.
• Perform skilled nursing interventions as ordered (medication management, wound care, etc.).
• Educate clients and caregivers on medications, treatments, safety, and disease management.
• Complete timely documentation of services and client status.
• Participate in Plan of Care reviews with RN and care team.
• Maintain weekly supervisory visit schedule with RN.
• Ensure compliance with GAPP, Medicaid, and agency requirements.
