Job summary

Flournoy Health Systems in Hall County, GA seeks a Licensed Practical Nurse to provide field-based clinical services for home and community clients. The LPN will perform supervisory visits, monitor client conditions, administer care per the Plan of Care, and collaborate with RNs and caregivers to sustain continuity of care.

Applicants must hold an active GA LPN license and a current BLS certification, have experience with medically fragile individuals, and be willing to travel to client homes

Qualifications

• Current, active Georgia LPN license.
• Current Basic Life Support (BLS) certification.
• Experience caring for medically fragile individuals.
• Knowledge of home and community-based service programs (ICWP, CCSP, SOURCE, GAPP).
• Ability to travel to client homes in the assigned territory.

Responsibilities

• Provide field-based nursing care, medications, and treatments per Plan of Care.
• Monitor client health, vital signs, and condition changes; report to RN.
• Perform skilled nursing interventions including medication management and wound care.
• Educate clients and caregivers on medications, treatments, and safety.
• Document services, client status, and changes promptly and accurately.
• Coordinate with RN and care team to review Plans of Care.
• Schedule weekly supervisory visits with RN.
• Complete supervisory and IDT visits and documentation.
• Escalate clinical or safety concerns to the RN or clinical leadership.
• Ensure compliance with GAPP, Medicaid, and agency requirements.
