Job summary

Flournoy Health Systems, Inc. is seeking a Licensed Practical Nurse (LPN) to provide field-based clinical services under the supervision of the RN.

The LPN will complete supervisory visits, monitor client conditions, apply nursing interventions, and ensure timely documentation in accordance with care plans and regulatory requirements. This role emphasizes caregiver support and continuity of care within the area’s pod model.

Qualifications

• Licensed Practical Nurse (LPN) with clinical nursing knowledge.
• Ability to assess patient conditions and respond to changes.
• Provide education to clients and caregivers about care plans.

Responsibilities

• Provide nursing care, medications, and treatments within Plan of Care.
• Monitor client health status, vital signs, and condition changes.
• Perform skilled nursing interventions as ordered (medication management, wound care, catheter care).
• Educate clients and caregivers on medications, treatments, safety, and disease management.
• Document services, status, interventions, and changes accurately and timely.
• Coordinate Plan of Care reviews and updates with RN and care team.
• Maintain the weekly supervisory visit schedule with RN coordination.
• Complete supervisory, IDT, and GAPP visits and documentation.
• Elevate clinical or safety concerns to RN or clinical leaders.
• Maintain compliance with Medicaid and agency requirements.
