Job summary

Flournoy Health Systems, Inc. seeks a Licensed Practical Nurse (LPN) to provide field-based clinical services for home care clients in assigned areas. You will perform supervisory visits, monitor client conditions, administer medications, and support care plans within your scope of practice.

Collaborate with RNs, caregivers, and operations to ensure timely documentation, safety, and quality of care, while maintaining regulatory compliance and participating in plan-of-care reviews.

Qualifications

• Current LPN license in Georgia or eligible to practice in Georgia.
• Experience in home health care preferred.
• Ability to perform supervisory visits and coordinate with RN teams.

Responsibilities

• Provide nursing care, medications, and treatments per Plan of Care within LPN scope.
• Monitor client health status and report changes to RN and care team.
• Perform skilled nursing interventions as ordered, including wound/catheter care and respiratory treatments.
• Educate clients and caregivers on medications, treatments, safety, and diagnosis management.
• Complete timely documentation of services, client status, and changes in condition.
• Participate in Plan of Care reviews and coordination with RN and interdisciplinary team.
• Develop and maintain the weekly supervisory visit schedule with RN.
• Complete required supervisory, IDT, and GAPP visits and documentation.
• Identify and escalate clinical, safety, or service-delivery concerns to the RN or clinical leader.
• Maintain compliance with applicable Medicaid and agency requirements.
