Primecare Home Care Services, Inc. is seeking a Licensed Practical Nurse (LPN) to provide field-based nursing care and ongoing support for clients receiving home-based services in Georgia. The role focuses on supervising visits, monitoring client conditions, and ensuring accurate clinical documentation.

Responsibilities include administering medications, wound and catheter care, patient education, and coordination with RNs and care teams. Travel to client homes within the territory is required.

Qualificações

• Current, active LPN license to practice in Georgia.
• BLS certification is required.
• Experience caring for medically fragile individuals is preferred.
• Knowledge of home and community-based service programs (ICWP, CCSP, SOURCE, GAPP) is a plus.
• Ability to travel to client homes within the assigned territory.

Responsabilidades

• Provide nursing care, medications, and treatments per Plan of Care.
• Monitor client health status, vital signs, and changes in condition and report to RN.
• Complete timely and accurate documentation of services provided and client status.
• Participate in Plan of Care reviews and coordination with the RN and interdisciplinary team.
• Develop and maintain the weekly supervisory visit schedule with the RN.
• Identify and escalate clinical, safety, or service-delivery concerns to the RN or clinical leaders.
