Job summary

Flournoy Health Systems is seeking an experienced Registered Nurse to provide clinical oversight and care coordination for clients receiving home and community-based services in Dougherty County, GA. The RN will lead initial assessments, develop individualized care plans, and support LPNs, caregivers, and operational teams to ensure quality care.

The role requires strong clinical supervision, familiarity with Medicaid and HIPAA regulations, and the ability to coordinate visits and documentation

Qualifications

• Current, active, and unencumbered Registered Nurse (RN) license to practice in the State of Georgia.
• Current Basic Life Support (BLS) certification.
• Demonstrated clinical competency in assessing and caring for medically fragile and/or complex-care individuals.
• Ability to complete comprehensive client assessments, identify changes in condition, and develop and maintain individualized Plans of Care.
• Ability to provide clinical supervision, direction, and education to LPNs, caregivers, clients, and families.
• Working knowledge of applicable home and community-based service programs, including SFC, Personal Support Services, GAPP, and other assigned programs.
• Valid driver's license, reliable transportation, and automobile insurance meeting applicable state and company requirements.
• Current TB screening.

Responsibilities

• Complete initial assessments and reassessments; maintain individualized Plans of Care.
• Provide clinical supervision to LPNs and caregivers.
• Coordinate supervisory and IDT visit schedules.
• Conduct supervisory visits for skilled clients and additional visits as needed.
• Review and sign off on clinical documentation to align with the Plan of Care.
• Manage physician POTs and coordinate clinical orders.
• Monitor complex medical conditions and coordinate interventions.
• Communicate with physicians, case managers, families, LPNs, caregivers, and teams to support care.
• Educate clients and caregivers about diagnoses, treatments, safety.
• Ensure services comply with Medicaid, HIPAA, and agency requirements.
• Provide direct in-field nursing support to address staffing shortages.
