Job summary

AmeriPharma seeks a dedicated Registered Nurse (RN) with home infusion expertise to join our team. You will perform in-home infusion visits, educate patients, and collaborate with pharmacy to ensure safe, effective therapy.

You will support patients undergoing TPN, IVIG, and IV biologics, maintaining detailed records and coordinating care under nursing supervision to meet treatment goals.

Qualifications

• RN license in good standing and graduate from an accredited nursing program.
• At least 2 years of home infusion experience.
• Experience with IVIG and IV biologics; CRNI certification preferred.
• Experience with peripheral IV starts, infusion therapy, and pediatric/adult phlebotomy preferred.
• Excellent interpersonal, time management and organizational skills.
• Strong written and verbal communication abilities.
• Ability to document thoroughly and coordinate with pharmacy and healthcare providers.

Responsibilities

• Conduct in-home infusion therapy visits with patients per plan of care.
• Prepare clinical/nursing notes and document interventions in records.
• Act as a clinical resource, collaborating with pharmacy departments.
• Maintain knowledge of drug side effects and interactions.
• Offer telephone and in-person patient education on therapy protocols and procedures.
• Ensure patients understand therapy plans, medications, and adherence importance.
• Serve as patient liaison among patient, family, and pharmacy to ensure seamless therapy.
• Perform head-to-toe assessments and initiate preventative and rehabilitative care.
• Develop care plans with interventions, supplies, and necessary services.
• Regularly evaluate progress with the team and revise plans as needed.
