Job summary

AmeriPharma is seeking a dedicated Registered Nurse (RN) with home infusion expertise to provide in-home infusion therapy and patient education. You will conduct visits, document interventions, and work closely with the pharmacy team to ensure safety, adherence, and optimal outcomes for patients undergoing therapies such as IVIG and IV biologics.

The role requires strong nursing judgment, excellent communication, and the ability to coordinate care across patients, families, and healthcare

Qualifications

• Graduate of an accredited nursing program with active RN license.
• 2+ years in home infusion and infusion centers.
• Experience with IVIG and IV biologics.
• CRNI certification is preferred.
• Strong communication and organizational skills.
• CPR and BLS certification current.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home infusion therapy visits with patients according to the plan of care.
• Prepare clinical/nursing notes and document patient interventions in records.
• Act as a clinical resource, collaborating with pharmacy departments.
• Maintain knowledge of drug side effects and interactions.
• Offer telephone and in-person patient education on therapy protocols and procedures.
• Ensure patients understand their therapy plans and adherence.
• Serve as patient liaison between patient, family, and pharmacy for seamless care.
• Perform head-to-toe assessments and initiate preventive care.
• Use health data to determine patient needs in collaboration with the team.
• Manage patient and family expectations regarding services and outcomes.
• Establish primary and secondary diagnoses and develop care plans.
• Regularly evaluate progress and revise care plans as needed.
• Perform other duties as assigned.
