Job summary

Wythe Hospice, part of UnitedHealth Group, is hiring a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients in home settings. You will evaluate and adjust care, collaborating with professionals to promote physical, psychosocial, and spiritual well‑being.

Required Virginia RN licensure and a valid driver's license; travel is expected within Grayson, Carroll, Wythe, Montgomery, and Pulaski counties. In-home visits, weekdays, with on-call rotation and benefits.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• 1+ years of clinical experience (preferred).
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient and family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and responses to care.
• Collaborate with patient/family, attending physician, and IDG members daily.
• Instruct and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes on patient condition.
