Job summary

Commonwealth Hospice, part of the LHC Group within UnitedHealth Group, is hiring a Hospice RN Case Manager in Danville, VA. This full-time role provides in-home visits to hospice patients, develops and implements the Plan of Care, and collaborates with an interdisciplinary team.

Requirements include Virginia RN license, valid driver's license and access to a vehicle or public transit, and 1+ year clinical experience; terminally ill experience is a plus.

Qualifications

• Current and unrestricted RN licensure in Virginia.
• Valid driver's license and access to a dependable vehicle or public transit.
• 1+ years of clinical experience preferred.
• Experience with terminally ill patients is a plus.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility.
• Develops and coordinates the Plan of Care with patient/family.
• Collaborates with attending physician and IDG for daily patient care.
• Provides in-home hospice nursing care and family education.
