Job summary

Advocate Health is seeking a Hospice Nurse Case Manager (RN) to coordinate comprehensive home hospice care across assigned patients. You will conduct assessments, develop care plans, provide skilled nursing care, and collaborate with the interdisciplinary team to support patients and families.

The role requires GA RN licensure, two years’ nursing experience, and travel across the service area. Full-time schedule with on-call rotation, mileage reimbursement, and strong benefits are offered.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current Georgia RN license required.
• Valid Georgia driver’s license and reliable transportation.
• Minimum of two years of nursing experience.
• Experience in end-of-life or hospice care preferred.

Responsibilities

• Conduct admission and routine visits, and assess patient needs.
• Develop and revise individualized patient care plans.
• Provide skilled nursing care and symptom management.
• Supervise home health aides and coordinate with caregivers.
• Maintain timely clinical documentation.
• Educate patients and families on disease progression and care at home.
• Collaborate with physicians, social workers, and chaplains in care planning.
• Support quality improvement and ensure regulatory compliance.
