Job summary

Wythe Hospice, a part of LHC Group and UnitedHealth Group family, seeks a Registered Nurse Case Manager for in-home hospice care in Virginia. You will evaluate patients, develop care plans, and coordinate interdisciplinary care with a focus on physical, psychosocial, emotional, and spiritual well-being.

Requirements include an active Virginia RN license, valid driver's license, and willingness to travel within Grayson, Carroll, Wythe, Montgomery and Pulaski counties.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle.
• VA: Experience with terminally ill required.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral.
• Identifies the patient’s/family’s physical, psychosocial, and environmental needs and reassesses as needed, no less than every fifteen (15) days.
• Initiates and coordinates the plan of care.
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
• Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.
• Instructs and supervises the patient/family in self-care techniques when appropriate.
• Maintains accurate and relevant clinical notes regarding the patient's condition.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
