Job summary

Ennoble Care is hiring a full-time Hospice Registered Nurse Case Manager to serve Gwinnett, GA and surrounding areas. You will work within an interdisciplinary team to assess, plan, and deliver hospice care that maximizes comfort for patients and supports families.

The role requires an active GA RN license and 1–2 years of hospice or professional nursing experience. Driving is required, with a focus on home-based patient care and caregiver education.

Qualifications

• Must be a licensed RN in Georgia.
• Preferably 1–2 years in hospice care.
• Strong ability to assess patient and caregiver needs and write detailed reports.

Responsibilities

• Provide skilled nursing assessments, planning, and care.
• Identify patient and caregiver needs and coordinate with interdisciplinary team.
• Maintain timely documentation in electronic medical records.
• Communicate with physicians and Hospice staff to coordinate care.
• Attend care conferences and contribute to the patient plan of care.
