The RN works with the Director of Nursing and an interdisciplinary team to provide skilled assessments, care planning, and comfort-focused support in patients' homes.

Candidates must hold a GA RN license, have 1–2 years of nursing experience (hospice preferred), and possess a valid driver's license with reliable transportation.

Qualifications

• GA RN license required.
• 1–2 years nursing experience; hospice preferred.
• Ability to assess patient and caregiver needs in home settings.
• Valid driver's license and reliable vehicle.
• Team player with interdisciplinary care approach.

Responsibilities

• Manage a patient caseload across nursing process phases.
• Coordinate with physicians and hospice staff to plan care.
• Document care in electronic medical records.
• Provide caregiver education and support.
• Participate in Interdisciplinary Group meetings.
• Handle off-hour guidance as needed.
• Attend patient care conferences.
• Comply with state and federal hospice regulations.
