Job summary

Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to serve patients in Gwinnett County, GA and surrounding areas. The RN will work with the interdisciplinary team to plan and provide skilled nursing care, focusing on comfort and supporting families.

Responsibilities include comprehensive assessments, care plan development, medication management, and coordination with hospitals, SNFs, and hospice units.

Qualifications

• Current license to practice as a registered nurse in GA.
• 1–2 years of professional nursing experience; hospice preferred.
• Ability to assess and respond to patient and caregiver needs in varied settings.
• Team player supporting an interdisciplinary care approach.
• Valid driver’s license with reliable vehicle and ability to travel year-round.
• Ability to work independently and in a multidisciplinary team.

Responsibilities

• Provide compassionate hospice nursing care to patients receiving GIP level of care.
• Perform frequent nursing assessments for pain, dyspnea, agitation, nausea, and other symptoms.
• Develop and update individualized care plans with the hospice team.
• Administer medications and titrate treatments per orders.
• Document patient responses and care coordination accurately.
• Educate patients/families about the dying process and symptom management.
• Coordinate care with hospital, SNF, or hospice units for continuity.
• Participate in discharge planning when symptoms stabilize.
• Maintain timely, compliant clinical documentation of interventions and outcomes.
