Job summary

Ennoble Care is seeking a Full-time Hospice Registered Nurse Case Manager to serve the Fulton/Cobb County, GA region. You will work within an interdisciplinary team to assess, plan, and provide nursing care to maximize comfort for patients and support for families.

The role requires RN licensure in Georgia, CPR certification, and a minimum of one year in nursing with hospice experience preferred. You will coordinate with physicians and caregivers, document care plans, and participate in care

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Must be a Registered Nurse licensed to practice in GA.
• One year of RN experience; Hospice experience preferred.
• CPR certification for patient care.
• Strong organization and time management skills.
• Must have a licensed driver with insured automobile.
• No exceptions to RN license; able to administer medications and nursing procedures as needed.

Responsibilities

• Manage an assigned patient caseload within the Hospice team.
• Coordinate care with physicians, Hospice staff, and other agencies.
• Document accurately in electronic medical records.
• Participate in Interdisciplinary Group meetings and care conferences.
• Provide support to patients and family caregivers, including education.
• Supervise NAC and assist during off-hours when needed.
• Maintain familiarity with Hospice policies and state/federal regulations.
