Job summary

Ennoble Care in Georgia is seeking a Full-time Hospice Registered Nurse Case Manager to join the interdisciplinary team serving Gwinnett and surrounding areas. You will perform skilled nursing assessments, care planning, and care coordination for patients at home.

The role requires current GA RN license, 1–2 years hospice or related experience, strong communication and documentation, and ability to work with caregivers. Competitive benefits and support for professional growth are provided.

Qualifications

Current Georgia RN license.
1–2 years hospice or related nursing experience.
Ability to assess and respond to patient and caregiver needs.
Strong observation and documentation skills.
Team player with interdisciplinary care approach.

Responsibilities

Manage patient caseload and the nursing process phases (assess, plan, implement, evaluate).
Coordinate care with attending physicians, Hospice staff, and other agencies.
Document accurately in the electronic medical records.
Participate in Interdisciplinary Group meetings and care planning.
Educate and support caregivers and family members.
