Job summary

Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to serve patients in Gwinnett County, GA and surrounding areas. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing and physicians to maximize comfort and health for patients and families.

Starting pay is $85,000 annually with a full benefits package, and candidates should have GA RN licensure, 1–2 years hospice experience, and a valid driver’s license

Qualifications

• Current license to practice as a registered nurse in the State of GA.
• 1–2 years of nursing experience; hospice care preferred.
• Ability to assess needs of patients and families and document clearly.
• Valid driver’s license, reliable vehicle, and year-round travel ability.
• Team-oriented with interdisciplinary care experience.

Responsibilities

• Provide compassionate hospice nursing care to General Inpatient level patients.
• Perform comprehensive and frequent nursing assessments for pain, dyspnea, and other symptoms.
• Develop and update individualized plans of care with the hospice team and family.
• Administer medications per physician orders and hospice plan of care.
• Document patient responses and coordinate with hospital, SNF, or inpatient hospice teams.
• Educate patients and families about goals of care and symptom management.
• Support discharge planning and transitions to Routine Home Care when stabilized.
