Job summary

Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to work within and around hospitals in Gwinnett County, GA and the surrounding areas. As a member of the interdisciplinary team, the RN Case Manager works under the direction of the Director of Nursing to provide skilled nursing assessments, planning, and care to maximize comfort and health for both patients and their loved ones.

Starting Pay: $85,000 annually.

Qualifications

• Current GA RN license.
• 1–2 years nursing experience, preferably in hospice.
• Ability to assess needs of patients and caregivers in varied settings.
• Team player with interdisciplinary team concept.
• Valid driver’s license and reliable vehicle, year-round driving ability.

Responsibilities

• Provide hospice nursing care primarily to General Inpatient (GIP) level.
• Perform comprehensive nursing assessments for pain, dyspnea, agitation, nausea, seizures, bleeding, and other symptoms.
• Develop, implement, and update individualized plans of care with the hospice team and family.
• Administer medications and treatments per orders and hospice plan of care.
• Evaluate patient responses and changes in condition.
• Communicate with hospice physician and team about changes and recommendations.
• Educate patients and families about the dying process and symptom management.
• Coordinate care with hospital/SNF/hospice inpatient staff for continuity and safety.
• Participate in daily evaluation and discharge planning.
• Maintain timely, accurate documentation demonstrating interventions and medical necessity.
