Job summary

Elite Hospice of Round Rock, part of LHC Group, is hiring a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients, collaborating with the IDG, physicians and families to support physical, psychosocial, emotional and spiritual well-being.

Primary responsibilities include initial nursing evaluations within 48 hours, care plan initiation and coordination, thorough documentation, and daily collaboration with the patient/family.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle.
• CPR certification or ability to obtain within 90 days.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs; re-assesses every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response.
• Collaborates with patient/family and IDG to provide daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding patient condition.
