Job summary

Caretenders Hospice in Ohio is seeking an RN Case Manager to deliver the plan of care for hospice patients and collaborate with the IDG to provide compassionate, high-quality care in patients' homes.

This PRN/inconsistent position requires an Ohio RN license, 1+ year of clinical experience, and access to a reliable vehicle; travel within a 50-mile radius is expected. Hours are Monday–Friday with on-call evenings and weekends.

Qualifications

• Current and active RN license in the state of practice (Ohio).
• Minimum 1 year of clinical nursing experience.
• Valid driver's license, auto insurance, and reliable vehicle or access to reliable public transportation.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify and reassess patient and family needs; update plan of care accordingly.
• Coordinate and implement the plan of care; document patient progress and outcomes.
