Job summary

Caretenders Hospice in Columbus, OH seeks an experienced Registered Nurse Case Manager to assume full nursing responsibility for hospice patients' care plans. You will evaluate, plan, implement, and adjust care in collaboration with a multidisciplinary team to promote physical, psychosocial, emotional and spiritual wellbeing.

Responsibilities include initial evaluations within 48 hours, ongoing reassessments, care coordination, and patient-family instruction.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identifies physical, psychosocial and environmental needs and reassesses every 15 days.
• Initiates and coordinates the plan of care and documents progress.
• Collaborates with patient/family, attending physician and IDG daily.
• Instructs patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
