Job summary

Caretenders Hospice is hiring an RN Case Manager to deliver comprehensive hospice care in patients’ homes within a 50-mile radius of the assigned service area. The role covers initial evaluation, care planning, and ongoing coordination with the care team.

The position is PRN with on-call evenings and weekends, requiring Ohio RN licensure and at least 1 year of clinical experience. Valid driver's license and reliable vehicle or public transit are required.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care; document goals and interventions.
• Collaborate with patient/family, physician, and IDG members in daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
