Job summary

National Church Residences Home Health division located in Central Ohio is seeking an RN Case Manager to join our team. This full-time role focuses on admissions and comprehensive care planning for seniors in a home health setting.

The RN will develop and implement individualized care plans, coordinate with physicians and the interdisciplinary team, and provide patient education and support throughout the care continuum. Occasional on-call weekends and holidays are required.

Qualifications

• Active RN license in Ohio.
• Minimum experience in home health nursing preferred.
• Excellent communication and documentation skills.

Responsibilities

• Develops and updates the individualized plan of care with client, caregiver and representative.
• Coordinates care with physicians and the interdisciplinary team and integrates orders.
• Administers medications and treatments as prescribed when required.
• Performs initial assessments and ongoing evaluations of client health status.
• Educates and mentors client and caregiver in home care tasks and plan of care.
• Coordinates discharge planning with interdisciplinary team when appropriate.
