Job summary

Gentiva Hospice is seeking a compassionate Registered Nurse Case Manager to provide in-home and facility-based hospice care. You will perform comprehensive assessments, develop individualized plans of care, and coordinate with an interdisciplinary team to deliver comfort and dignity to patients and families.

The role emphasizes clinical excellence, clear communication, and adherence to regulatory standards while supporting caregivers with education and bereavement guidance.

Qualifications

• RN license in the state of employment.
• Minimum 1 year RN experience in hospice, palliative care, home health, or acute care.

Responsibilities

• Perform comprehensive RN assessments and ongoing reassessments in patient homes or facilities.
• Deliver skilled nursing interventions aligned with care plans and symptom management.
• Develop and update Plans of Care with visit frequency to meet patient needs.
• Coordinate care with physicians, LPNs, CNAs, social workers and other IDG members.
• Educate patients and caregivers on disease process, medications, end-of-life care, and self-care.
