Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to perform in-home health evaluations for members in their own homes. Each visit lasts about 45 minutes and includes history review, physical assessment, and coordination with the health plan.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations to health plan members.
• Perform a 45-minute visit including history review and physical assessment.

• Travel locally to member homes across licensed states; virtual visits as needed.
